Home From Hospital Care
Coming home from hospital can be a challenge and a worry. Cleansed Vision has a service designed to help an individual have a smooth and safe discharge home from the hospital. We will work in tandem with the hospital discharge team. The package can be a 3-hour one-off package or a temporary care period while the person regains their strength and confidence.
